By signing this form you agree to the following conditions:

• Your dog/dogs will remain on the ground level of the house and not be
  allowed upstairs.
• Your dog/dogs will not go on the furniture.
• Your dog/dogs will not be left in the cottage unattended.
• Your dog/dogs will be cleaned thoroughly and be free of sand before entering
  the cottage.
• Your dog/dogs will have been treated for the prevention of fleas.
• Your dog/dogs will not be a nuisance to the neighbours.

Declaration
I have read and accept the Booking Conditions for a dog/dogs at Sailfish Cottage . I
agree to be held responsible for any damage that my dog/dogs may be responsible
for. The damage deposit that I have paid extends to any damage that may be
caused by my dog/dogs. If the cost of damages exceeds my deposit I will also be
responsible for the additional amount.
